Hillsborough County Health Care Plan a comprehensive free managed care program for Hillsborough County residents with limited income and assets who do not qualify for other health care coverage, including Medicare and Medicaid. The program is funded by a special sales tax and administered by Hillsborough County. There are no premiums or copays for services covered by the Hillsborough County Health Care Plan.

It’s Annual Renewal Time!

We have begun our annual renewal process of collecting all new FREE AND REDUCED MEAL APPLICATIONS AS OF SEPTEMBER 1ST FROM ALL ENROLLED CHILDREN IN YOUR CENTERS.
As of September 1st the old PDF Form containing the link to EMA (Electronic Meal Application), has been replaced with a NEW PDF FORM CONTAINING A NEW LINK THAT YOU MUST SEND TO ALL OF YOUR ENROLLED CHILDREN’S FAMILY TO COMPLETE FOR THE NEW FISCAL YEAR. All current Free and Reduced Applications used for the past year have expired as of September 30th, 2021.
This New PDF is located in the same location as the previous PDF on our web site in the Members Only Portal where you sign into your online recordkeeping, located in the EMA TAB.
PLEASE DOWNLOAD AND SAVE THE ENTIRE PDF FORM AS IT CONTAINS THE REQUIRED PARENT LETTER AS WELL AS THE LINKS IN ENGLISH AND SPANISH TO COMPLETE THE FREE AND REDUCED APPLICATION. THEN EMAIL THE PDF TO ALL ENROLLED CHILDREN’S HOUSEHOLD TO COMPLETE THE FREE AND REDUCED APPLICATION FORM THE OLD PDF LINK THAT YOU HAVE SENT PREVIOUSLY WILL NO LONGER WORK AS OF SEPTEMBER 1st.
If you have saved a version of this old PDF, please delete it and start sending the NEW PDF to your families to complete. As in the past years, we will need all new applications completed before the end of October in order to process our October Claim.
The new completed applications will be valid for the new fiscal year starting September 1st 2021 through September 30th, 2022. If you have any questions please contact any of our staff to assist you.
IMPORTANT DATES – Mark Your Calendar So You Don’t Miss Out on Monthly Reimbursements

PLEASE PUT THESE DATES IN YOUR CALENDARS SO YOU ARE IN COMPLIANCE AND DON’T MISS OUT ON GETTING YOUR MONTHLY REIMBURSEMENT! YOUR CLAIM DOCUMENTATION IS DUE ON OR BEFORE THE DUE DATES BELOW. YOUR CURRENT SEPTEMBER CLAIM IS DUE IN OUR OFFICE ON OCTOBER 5TH, 2021.
Based on your CCFP agreement*, your claims must be IN OUR OFFICE on the below dates; however, if your claim is postmarked by the 3rd we will honor it as on-time even if we receive it after the due date. Also note that beginning with the September Claim (due October 5th), we will no longer honor claims that are over 30 days past due. These FINAL late claim dates are noted below as well.
*PER ITEM F. IN YOUR CCFP AGREEMENT YOU HAVE AGREED TO SUMBIT…the previous month’s CCFP claim documentation and supporting records in time to be received by the sponsor by the fifth (5th) day of each month. Records to be sent include monthly menus; monthly Meal Count Records; monthly expense documentation; and Enrollment, Child Participation, and/or Infant Feeding Forms for any children enrolled during the
claim month. If the fifth (5th) day of the month falls on a weekend or holiday, these records must be received by the sponsor on the first business day following the weekend/holiday.
** ELECTRONIC CLAIM INFORMATION NOTE: All EMA applications and CNCORK Recordkeeping information (enrollment, meal counts and attendance) for each month’s claim must be entered DAILY but must be verified and complete by 10pm on the last day of the month in CNCORK.
***QUICK TURN AROUND MONTH: This is a quick turn around month due to a holiday prior to the claim due date…plan ahead and be sure to have all of your receipts collected and paperwork in order throughout the month so you are not late submitting on this month!
NEVER BE LATE AGAIN!! TIPS FOR EFFORTLESS AND ALWAYS ON-TIME CLAIMS:
UPON ENROLLMENT OF A CHILD: Include the EMA application in your enrollment process for new children. Just like you wouldn’t enroll a child without their shot records, don’t enroll a child without either verifying the parent has completed the EMA form, infant feeding form or collecting a Child Participation Form
DAILY: Enter attendance, meal counts and newly enrolled children in CNCORK – DO THIS DAILY!!
WEEKLY: On the first day of every claim month address an envelope to CORNERSTONE FAMILY MINISTRIES 2801 N 17th Street then weekly place your grocery receipts, edited final menus for the prior week, and Child participation and infant feeding forms collected that week into the envelope.
MONTHLY: On the last day of the month add your School Readiness information to the envelope you have been adding receipts, menus, infant feeding forms and child participation form to throughout the month. Copy all of the contents of the envelope for your records. Seal the envelope, apply postage and mail it to us on the 1st of the month following the claim!
Grab-N-Go Meal Program

Cornerstone’s own Rosa Valdez Early Learning Center & Lab School launched its GrabNGo Program providing weekly CCFP meals packages and literacy items to families. Each CCFP meal package included 5-days worth of breakfast, lunch and snack to registered families with children ages 12 yrs. and younger.

Cornerstone Childcare Heroes Parade
It all begins with an idea.
This Parade is all about YOU so we want to see all of you there! Our vendors, Board members, staff and volunteers will be on-hand to shower you with thanks and gifts in appreciation for all you do to help us serve over 16,000 children each year. We could not feed so many children without you. What a blessing you are!
As most of you know, United Methodist Women from across the nation donated thousands of masks to Cornerstone Family Ministries so that we could help you safely reopen your centers. Several of these women will be at the parade on September 8th to celebrate you and bless you with books they are currently collecting for your centers. Many of you have sent thank you notes to us but truly we couldn’t have been so generous without this group of women….so we thought it might be a fun to surprise them at the parade by giving you a chance to thank them directly. Here’s what we have in mind:
Put A Large Sign in Your Front Dash Board with your Center Name (so we know it’s you for your prizes, awards and gifts)
Decorate your cars with signs saying “Thank You UMW for Our Masks & Books”
Have your teachers and children make handmade cards on folded 8.5×11 paper with drawings and photos saying THANK YOU for the masks. Bring the cards to the parade in a gallon zip lock bag. We will mail the cards to the many elderly women who have been isolated during COVID and who sewed masks from home for your centers…the cards will be a great blessing to them!
Make some noise as you drive up and shout “Thank You United Methodist Women!”
