Enrollment Process

Directions for Completing the Online Enrollment Application

You MUST download and review the Parent Handbook prior to starting the enrollment application. This file includes a parent letter, parent handbook, Influenza brochure, and Know your Child Care Center brochure.

This enrollment application will take approximately 30 minutes to an hour to complete. Once you start, you can NOT save and return to this application. You MUST complete it in its entirety once you’ve begun. We will contact you within 48 business hours upon receipt of your packet.

Before accessing the electronic enrollment application, please have the following files ready to be uploaded into the application:

  • Most Recent Shot Record

  • Most Recent Physical Record

  • VPK Voucher

  • School Readiness Voucher (If applicable)

Please also have the following information ready to be entered into the application:

  • Contact Information the Child’s Physician

  • Contact Information for Individuals Authorized to Pick Up Child

  • Contact Information for Parent/Guardian

  • Child’s insurance information (if applicable)

DEBE descargar y revisar el Manual para padres antes de comenzar la solicitud de inscripción. Este archivo incluye una carta para padres, un manual para padres, un folleto sobre la influenza y el folleto Conozca su centro de cuidado infantil.

Esta solicitud de inscripción tardará aproximadamente entre 30 minutos y una hora en completarse. Una vez que comience, NO podrá guardar y volver a esta aplicación. DEBE completarlo en su totalidad una vez que haya comenzado. Nos comunicaremos con usted dentro de las 48 horas hábiles posteriores a la recepción de su paquete.

  • Registro de disparos más reciente

  • Registro físico más reciente

  • Cupón VPK

  • Cupón de preparación escolar (si corresponde)

Tenga también lista la siguiente información para ingresarla en la solicitud:

  • Información de contacto del médico del niño

  • Información de contacto de las personas autorizadas para recoger al niño

  • Información de contacto del padre / tutor

  • Información del seguro del niño (si corresponde)

Tuition Rates

The Rosa Valdez Center is able to offer quality care at affordable prices because of generous donations from our fundraising efforts that offset every child’s tuition.  The Rosa Valdez Center accepts VPK and School Readiness Vouchers and never charges additional fees over and above the School Readiness Rate.  The Center is also  part of the Eckerd Early Head Start program providing income eligible childcare for up to 20 children.  Cornerstone also provides a limited number of Childs Chance for Change Sliding Scale Scholarships each year to income eligible families who are working and going to school but do not qualify for any other assistance.
